How much bacteriostatic water for 10 mg retatrutide

The water to add to a 10 mg retatrutide vial so your dose is the number of units you want to draw.

Ozempic, Wegovy, Mounjaro and Zepbound pens give the fixed doses on their labels and are not marked in units: see the label dose charts.

How the numbers are found

  1. The units you want are a volume: units ÷ 100 = mL per dose.
  2. That volume must hold your dose, so the concentration is the dose ÷ that volume.
  3. Water is the peptide in the vial ÷ that concentration.
  4. With a pen, all the mix goes into the cartridge, so the water is at most what the cartridge holds; each dose also uses a priming shot.

Water for a 10 mg vial

No amount of water is the right one: the water sets the strength, and the strength sets the units each dose takes. 1 mL makes 10 mg per mL, 2 mL makes 5 mg per mL, 3 mL makes 3.33 mg per mL. Pick the water that puts your dose on a line of your syringe and keeps it within one syringe.

A 10 mg vial: the doses of the phase 2 trial (Jastreboff et al., NEJM 2023) in units on a U-100 syringe for each amount of water. Over 100 units does not fit a 1 mL syringe.

The doses are those of the retatrutide phase 2 trial, given once a week: arms of 1, 4, 8 and 12 mg, four of the six stepping up from 2 or 4 mg. The water changes the units per dose, not the number of doses: a 10 mg vial holds less than one 12 mg dose. The retatrutide dosage calculator lists the arms and their results.

Mixing the vial

  1. Find the water: read it from the table, or enter your dose and the units you want per dose above.
  2. Draw that much water into a syringe and inject it into the vial with the stream aimed at the glass wall, not at the powder.
  3. Swirl the vial gently until the powder has dissolved. Do not shake it.
  4. Write the concentration and the date mixed on the vial: Print label above prints the concentration with a line for the date.

Steps 2 and 3 are the handling the Serostim label gives for its freeze-dried somatropin; retatrutide has no FDA label. Sterile and bacteriostatic water are compared on the reconstitution calculator.

The example in the calculator. A 10 mg vial, a 2 mg dose, 40 units per dose. 40 units is 0.4 mL, so the mix must hold 2 mg in 0.4 mL: 5 mg per mL. 10 mg at 5 mg per mL is 2 mL of water, and the vial holds 5 doses of 2 mg.

Is retatrutide FDA-approved?

No. Retatrutide is an investigational drug in phase 3 trials, and the FDA has not approved it for any use.

FDA
Not approved, and cannot be used in compounding under federal law. FDA has warned companies selling it labelled “for research purposes” or “not for human consumption”.
Advisory committee
Not on the agenda of the Pharmacy Compounding Advisory Committee (meetings of October and December 2024 and July 2026), and not among the substances nominated for compounding under section 503A (FDA list updated 2026-05-14).
WADA
Not named on the 2026 Prohibited List or on the 2027 List. Its section S0 prohibits at all times any substance with no approval for human therapeutic use, such as drugs in clinical development.
Trials
Phase 3 (TRIUMPH): TRIUMPH-1 to TRIUMPH-4 completed, TRIUMPH-5 to TRIUMPH-9 active, not recruiting; no results posted on ClinicalTrials.gov.

Questions

How much bac water for 10 mg of retatrutide?

As much as puts your dose on a line you can read. With 2 mL the vial is 5 mg per mL and the trial’s 2 mg step is 40 units; with 1 mL it is 10 mg per mL and 2 mg is 20 units; with 3 mL, 60 units.

How much does one unit hold?

With 1 mL of water, 0.1 mg (100 mcg); with 2 mL, 0.05 mg (50 mcg); with 3 mL, 0.033 mg (33.3 mcg). A unit is 0.01 mL on a U-100 syringe.

How many doses are in a 10 mg vial?

10 doses of 1 mg, 5 of 2 mg, 2 of 4 mg and 1 of 8 mg, whatever the water. A 12 mg dose needs more than one 10 mg vial.

Is retatrutide FDA-approved?

No. As of 2026-10-01 the FDA says it is not a component of any FDA-approved drug and cannot be used in compounding under federal law.
